The Lowcountry Integrative Center is the specialty division of Lowcountry Wellness Center. LIC provides advanced evaluation and ongoing care for patients with Lyme disease, tick-borne illnesses, mold-related illness, Long COVID, autoimmune conditions, complex hormone imbalances, and other medically complex chronic conditions.

Unlike traditional primary care, LIC is intentionally designed as a limited-enrollment specialty program. This allows Dr. Penni, APRN to provide the time, thoughtful investigation, collaboration, and individualized attention that complex patients often need.

Our approach combines evidence-informed medicine with a root-cause philosophy while recognizing that every patient’s story is different and every treatment plan should be too.

Comprehensive Chronic Illness Evaluation

A two-hour specialty evaluation with Dr. Penni, APRN

Before your appointment, our team carefully reviews your completed health questionnaire, previous laboratory testing, imaging, and available medical records so the time with Dr. Penni, APRN can be used as effectively as possible.

During your evaluation, Dr. Penni, APRN will:

  • Review your complete medical history
  • Analyze previous diagnostic testing and treatments
  • Identify potential root causes and contributing factors
  • Discuss additional testing when appropriate
  • Develop an individualized treatment strategy
  • Create your initial care roadmap

This evaluation provides the foundation for your care and helps determine the most appropriate path forward.

Determining Your Best Care Path

Every specialty patient begins with the $895 Comprehensive Chronic Illness Evaluation. Following your evaluation, Dr. Penni, APRN will recommend the ongoing care model that best fits your medical needs and long-term health goals.

Not every patient requires ongoing monthly specialty care.

Some patients benefit from periodic consultation while continuing care with their local primary care provider or specialist.

Consultation visits are scheduled as needed and do not include secure messaging, ongoing care coordination, or between-visit treatment management.

PRIMARY CARE

Continued Care With a Primary Care Provider

Some patients may be better served through Direct Primary Care or by continuing care with their existing primary care provider.

When appropriate, Dr. Penni, APRN may provide specialty recommendations while your primary care provider continues managing routine healthcare needs.

  • Continue routine primary care elsewhere

  • Maintain clear separation between primary and specialty care

INDIVIDUALIZED TREATMENT

Advanced Therapies When Clinically Appropriate

Treatment plans are based on each patient’s history, clinical findings, previous testing, and individual health goals.

Advanced therapies, including Q-REstrain™, may be recommended when clinically appropriate and incorporated into a patient’s individualized treatment plan.

No single therapy is appropriate for every patient, and no treatment is recommended simply because it is available.

Personalized care means the treatment plan follows the patient, not the other way around.
WHY WE LIMIT ENROLLMENT

Quality, Not Quantity

Complex chronic illness cannot be effectively managed through rushed appointments.

LIC intentionally maintains a limited patient panel so every patient receives the time, thoughtful investigation, responsiveness, and continuity that medically complex care requires.

Limiting enrollment also allows Dr. Penni, APRN to remain deeply involved in each patient’s care while continuing to mentor the clinical team and advance the specialty services offered through Lowcountry Wellness Center.

Dr. Penni, APRN provides LIC specialty evaluations, advanced treatment planning, and ongoing management for patients enrolled in the Comprehensive Chronic Illness Program.

Her approach is shaped by years of clinical experience, specialized training, and a deep understanding of how difficult it can be to live with symptoms that have been dismissed, misunderstood, or treated in isolation.

All Comprehensive Chronic Illness Evaluations are conducted directly with Dr. Penni, APRN.

When I founded the Lowcountry Integrative Center, my vision was simple: to create the kind of specialty practice I wished more patients with complex chronic illness could find.

Patients living with Lyme disease, tick-borne infections, mold-related illness, and other chronic conditions deserve more than symptom management. They deserve time to be heard, thoughtful investigation, and a clinician willing to look beyond the obvious.

My goal is not simply to help you feel better. It is to partner with you in identifying the factors contributing to your illness and to develop a personalized treatment plan that supports meaningful, long-term healing.

To preserve that level of care, I intentionally limit the number of patients I accept into the Comprehensive Chronic Illness Program. This allows me to remain deeply involved in each patient’s care while continuing to mentor our clinical team and advance the specialized services offered through Lowcountry Wellness Center.

I consider it a privilege to walk alongside my patients through some of the most challenging seasons of their health, and I look forward to the opportunity to partner with you.

What to Know Before You Begin

No. Every prospective LIC patient begins with secure enrollment, a New Member Health Questionnaire, and a nurse consultation with Hailie, LPN when indicated. Patients who may be appropriate for LIC then complete a Comprehensive Chronic Illness Evaluation with Dr. Penni, APRN. Ongoing program enrollment is based on clinical need and is not automatic.

No. Some LIC patients receive primary care through Lowcountry Wellness Center, while others continue care with an outside primary care provider. The best arrangement depends on your individual needs.

No. The Comprehensive Chronic Illness Program is recommended only after Dr. Penni, APRN completes the evaluation and determines that ongoing specialty management is appropriate.

Many follow-up visits may be completed virtually when clinically appropriate. The type of visit recommended depends on your condition, treatment plan, and current medical needs.

No. LIC is a specialty chronic illness practice. Routine preventive care, annual physicals, newborn and pediatric care, sick visits, and management of straightforward primary care concerns are provided through Lowcountry Wellness Center or another primary care practice.

No. Patients whose needs clearly align with straightforward Foundational Primary Care can move forward without a consultation. Hailie, LPN contacts patients when the questionnaire suggests that Health Optimizer or LIC may be the better fit.

Complex Care Is a Process

We believe in being honest about what specialty chronic illness care may require.

There is rarely a one-visit solution.

Complex illness often requires ongoing evaluation, treatment adjustments, and time.

Additional testing may be recommended.

Testing is based on clinical need and is not automatically included in program fees.

Care is highly individualized.

Two patients with the same diagnosis may receive very different treatment recommendations.

The financial commitment may extend beyond the evaluation.

Medications, supplements, testing, procedures, and outside services may involve additional costs.
